Notes:

1. Supporting image format for upload is jpg, jpeg only.

2. Supporting file format for upload is pdf, doc only.

3. Attachment can be of maximum 2 MB size

• This is the final page of the application form.

• It includes the necessary supporting documents which are to be provided & also the DD made for the proposal.

• Firstly the proposed school front side image with compound wall & gate of the school is to be uploaded. Click the Browse button to select school image from your system and then click Upload .The uploaded image can be removed by clicking the Remove image option provided for it in front of the image.

• Details of the Trust/Body account should be provided with account balance on application date and account proof document. More than one entry can be done for the account details if applicable.

• The attachments section include required documents

• Select/choose availability of the required documents by selecting "Yes” or "No" provided.

• All the necessary documents required in the fields should be scanned and the images should be "browsed" by providing proper link and then uploaded to the page by clicking "Upload" button against all the required fields as shown below.

• Attached documents can be viewed by clicking "View Attachment" button provided against all the fields, similarly can be removed by clicking "Remove" option against the fields.

• Any comments can be written against the required attachments in the space provided as shown.

• After attaching all the necessary copies, details of the Demand Draft made for the payment should be provided as shown regarding Bank name, DD number, DD date etc.

• After filling the form completely, applicant is requested to carefully read the "Self Declaration” terms at the bottom of the form and then check the box as shown above.

• Finally click on to complete & post the application.

• After successful submission of application, system will generate application number for your application that will be mail to your Email-id.

•   Use that Application no to check your application status online.
